Governance Maturity Assessment (Beta Version 1)

This free beta tool helps HCBS, LTSS, IDD, behavioral health, aging services, home care and community-based organizations assess governance capability, executive oversight, organizational accountability, risk management and assurance maturity.

The assessment generates an overall governance maturity score, detailed domain findings, assurance recommendations, risk oversight priorities, board effectiveness prompts and a staged governance improvement roadmap.

This tool provides general governance and quality improvement guidance only. It does not replace professional judgment, legal advice, state requirements, Medicaid requirements, CMS requirements, managed care contract requirements, accreditation standards, adult protective services procedures, clinical advice, information governance requirements or organizational policy.


Assessment Scale

Rate each statement using the position that most accurately reflects current practice.

  • 1 — Reactive: arrangements are informal, inconsistent or largely absent.
  • 2 — Emerging: some arrangements exist, but implementation and evidence are limited.
  • 3 — Defined: arrangements are documented and generally understood, although consistency varies.
  • 4 — Assured: arrangements are embedded, monitored and supported by reliable evidence.
  • 5 — Leading: governance is proactive, outcome-focused, continuously improved and demonstrably effective.
