Impact Insights Digital Tools is a growing collection of practical tools designed to help HCBS, IDD, behavioral health, home care and human services providers move beyond guidance and into real-world application.
The aim is to transform complex areas such as community impact, quality assurance, risk management, value-based outcomes, system sustainability, innovation and digital transformation into usable resources that support day-to-day decision-making, planning and improvement.
This section sits alongside the wider Value, Impact & System Sustainability Knowledge Hub and the Innovation, Pilots & Emerging Models Knowledge Hub, helping organizations move from reading best practice to implementing it through structured frameworks, reporting tools and operational resources.

A practical beta tool designed to help HCBS and human services providers create structured community impact reporting frameworks. The tool generates suggested KPIs, evidence sources, funder-ready reporting prompts and governance recommendations that can support grants, contract reviews, board reporting, MCO discussions and ongoing performance management.

Why Digital Tools?
HCBS, disability, behavioral health and human services providers are increasingly expected to evidence impact, governance, quality, risk management, workforce planning, health equity, community benefit and system value in clearer and more structured ways. Digital tools can help translate guidance into practical outputs that support grants, contract reviews, audits, payer discussions, board assurance and internal improvement.
The first tools focus on low-risk, high-value planning areas such as community impact reporting, evidence readiness and positive risk enablement. Over time, this section may expand into more advanced planning tools, predictive analytics, payer intelligence, governance support systems, operational dashboards and technology-enabled assurance models.
As the Impact Insights Knowledge Hub continues to grow, the Digital Tools section aims to bridge the gap between information and implementation by providing practical resources that organizations can use within everyday service delivery.
